In order to facilitate administrative processes, below are direct access links to useful information on some of these processes:

• Authorisations: Remember that for certain diagnostic tests, treatments and surgeries, the authorisation of the insurance company is essential. Contact your insurer.

With respect to clinical documentation, download the request form here. Fill it in in full and deliver the printed copy, following the instructions inside the document, to the Clinical Documentation department. If handled by another person, also complete the Clinical Documentation Delivery Authorisation and bring the identity documents listed in the request document with you.
